Authorization for Background Check

Class Home Care LLC is committed to maintaining a safe and trustworthy environment for its clients and employees. As part of our employment process, we require background checks for all employees, including criminal history, employment verification, and other records relevant to the caregiving profession.

By signing this form, you authorize Class Home Care LLC and/or its authorized agents to obtain background reports, including but not limited to:

- Criminal background checks (local, state, and federal)
- Sex offender registry checks
- Verification of previous employment and references
- Social Security number verification
- Driving record (if job duties include driving)
- Any other reports necessary to assess your eligibility for employment

This information will be used solely for employment purposes and will be handled in accordance with all applicable privacy laws and regulations.

Disclosure

I understand that:

- This authorization is valid for the duration of my employment unless revoked in writing.
- I have the right to request information about the nature and scope of the background report conducted.
- I will be provided with a copy of the report and a summary of my rights under the Fair Credit Reporting Act (FCRA), if applicable.

Consent and Signature

I hereby authorize Class Home Care LLC and its designated agents to obtain and use information from any background screening service or relevant agency for employment purposes. I release Class Home Care LLC, its agents, and any individual or entity providing information from all liability in connection with the background check process.
